The complete list of eight documents that an applicant must submit upon admission: from the certificate or diploma of education to medical certificates.
Upon admission, the applicant submits a complete package of eight documents. Please note: for several items, both the original and a copy are required.

01Education and testing
Certificate or diplomaCertificate or diploma of education with transcript (original and copy).
Testing certificateCertificate of completion of testing.
02Personal documents
3×4 photographsPhotographs 3 x 4 — 6 pcs.
ID cardA copy of the identity card.
Documents confirming benefitsDocuments confirming entitlement to benefits.
Registration certificate or military IDCopy of the certificate of registration with a military recruitment office or military ID (for full-time study, copy).
03Medical certificates
Form 075-U certificate and fluorographyMedical certificate form 075-U, fluorography image.
Vaccination certificateMedical vaccination certificate (form 063) for full-time study (copy).
